Registration for the 2010 conference is now open.  Registration is $50 including continuing education credits. 

Visa and Master Card are accepted.  Charges will appear as a charge to "Health Watch USA"  

Payment of the Registration Fee of $50 May Be Done By Five Methods:
Note Fee is Non-Refundable but is Transferable.   

#1  Online Credit Card:  This is a TWO Step Process.  You must submit a separate form for each registrant and also Pay by Credit Card -- Be sure to enter the correct payment amount of $50 per registrant.  These two areas will each open in a separate window. 

  

#2  Credit Card By Phone:  Call 606-425-7278 and ask for Cathy.    

#3  Credit Card By Mail:  Download and Print Credit Card Payment Form, fill out "Credit Card Information Form" and send to:
