Registration for the 2019 conference is now open.  Registration is $25 including continuing education credits.  Any problems please call 606-425-7278.

Payment and Registration:  The Registration Fee of $25  for the conference, including snacks and dinner by Cosi.  

May Be Done By TWO Methods:  Note Fee is Non-Refundable but is Transferable.   

	#1  Credit Card or PayPal:
	Make sure you tell us who the registration is for and the email address you used to make the Credit Card or PayPal Payment! IMPORTANT:  If you do not have a PayPal account, select the "Don't have a PayPal Account" option and enter your name and billing address.  You will be directed to a Credit Card entry screen.

	Or you can also send a single payment for a large number of multiple registrants via PayPal to healthwatchusa@gmail.com     Besure to include the registrants' names.

#2  Credit Card Over the Phone or By Snail Mail: 

Visa, Discover, American Express and Master Card are accepted.  Charges will appear as a charge to "Health Watch USA"
